Miller-Meeks Announces $209 Million for Iowa as Part of Historic Rural Health Transformation Program

December 29, 2025

IOWA — Today, Congresswoman Mariannette Miller-Meeks (IA-01) proudly announced that Iowa will receive $209,040,064 in federal funding as part of the Rural Health Transformation Program, the largest federal investment in rural health care in American history. This groundbreaking initiative, established under the Working Families Tax Cuts legislation passed by House Republicans and signed by President Trump, represents an unprecedented $50 billion commitment to revitalizing rural health care systems across the country.

"This is a transformational moment for rural Iowa," said Miller-Meeks. "Access to high-quality health care should never depend on your zip code, and today, we’re delivering on that promise. This $209 million investment will bring top-tier care closer to home for families, seniors, and veterans in every corner of our state. I fought hard to ensure Iowa received its fair share, and I’m proud to see that fight pay off for the people I serve."

The Rural Health Transformation Program is designed to support states in expanding access to care, modernizing outdated facilities and technologies, strengthening the rural health workforce, and deploying innovative care models. Iowa’s share of the funding will be used to:

  • Strengthen and expand rural health care providers and staff
  • Modernize rural hospitals and clinics with new technology and infrastructure
  • Improve emergency medical services and mental health access in underserved communities
  • Support community-based care delivery models that meet the unique needs of rural populations

This achievement builds on Miller-Meeks' long-standing commitment to improving health outcomes for rural communities. A physician, veteran, and strong advocate for rural Iowa, Miller-Meeks has consistently prioritized rural health in Congress and has sent multiple letters to leadership at the Centers for Medicare & Medicaid Services (CMS) and state leadership urging full and fair consideration for Iowa's RHTP application.

BACKGROUND:

  • In August & September 2025, Miller-Meeks hosted roundtables with hospital leaders across Southeast Iowa to gather input and advocate for targeted relief.
  • In October, Miller-Meeks led the Iowa delegation in sending a letter to Governor Kim Reynolds and Iowa HHS Director Larry Johnson expressing strong support for the Rural Health Transformation Program and outlining several guiding principles for consideration as Iowa prepares its application.
  • In November, Miller-Meeks led the Iowa Congressional Delegation in sending a letter to CMS Administrator Dr. Mehmet Oz showing strong support of Iowa’s application for the program.

"This is the result of listening to rural Iowans and delivering real results," Miller-Meeks continued. "This funding will not only save lives but also strengthen our rural communities for generations to come."
